South East Bulloch (Brooklet) Football Games Played at High School located Next to Middle School

Take Hwy. 80 towards Statesboro turn left onto Lee St.  Lee St. is just pass the Discount Grocery Store after entering the town of Brooklet.  Follow Lee St. to Brooklet – Denmark Hwy, turn left, the school is located at 9124 Brooklet-Denmark Hwy.   (approx 2 miles) (842-9555)  Special Note: The middle school plays at the High School football field.

Richmond Hill Middle School Football Games Played at High School located Next to Middle School

Interstate 95 South to exit 87 (Hwy 17).  Turn right onto Hwy 17, go ½ mile, turn right on to Harris Trail Rd. approx. 1 ½ mile school is on left (665 Harris Trail 459-5130).  Special Note: The middle School plays at the High School football field.

Langston Chapel Football Field (Julia P. Bryant Stadium

As you approach Statesboro via Highway 80 continue to the BY PASS TRAFFIC LIGHT (Landmarks: IHOP, lOWE'S ETC.). From this point (don't include this traffic light) start counting traffic lights as you continue down Highway 80.  Start counting traffic lights till you reach number 10.  This will be STOCKYARD ROAD (Landmark: Parkers).  Turn left at this traffic light and continue for approximately 1.5 miles.  Julia P. Bryant Elementary School will be on the left.  The football field is behind the school and you can park your bus in the bus parking area on the front and walk down to the field behind the school.

Screven Middle School (Sylvania) Football Games played at High School ( See Directions Below)

 Hwy 21 to Sylvania, in downtown Sylvania, turn right onto Ogeechee Rd. Travel ½ mile and then turn left onto Friendship Rd, school is on right.  (126 Friendship Rd., 912-564-7468)j Special Note: Middle School Plays at the High School Football Field. See directions below.

Hwy 21 towards Sylvania, turn right onto Sylvania by-pass.  Travel approximately ½ mile and take a right on Halcyondale Rd.  School is down on the left. (110 Halcyondale Rd., 912-564-7836)

William James Middle School (Statesboro)

Hwy. 80 through Statesboro, once on the other side of Statesboro the school will be on your left. (18809 Hwy. 80 West Statesboro, 489-5916)

Burke County Middle School (Waynesboro)   Football Games Played at High School Field

Hwy 21 to Sylvania, get on North 301 bypass.  Stay on 301 until you get to Hwy 24, turn left on Hwy 24 (north or West). Take 24 to Waynesboro; turn Right onto South Side Dr. (356 Southside Dr 706 554-3532) Special Note: Middle School Football Games played at High School.

Physical & Parent Forms

Physical Information:

In order to participate in any sport, you MUST have a current physical on file. SEMS will be using the DRAGONFLY MAX app. Athletic forms must be completed electronically and uploaded on DRAGONFLY before a student can participate in any Athletics. 

• The physical exam forms and medical eligibility, which must be completed by a physician, may be printed below. These forms must be uploaded directly to the DRAGONFLY MAX app. 
• The travel/insurance form, cardiac awareness and concussion forms can be completed electronically on DRAGONFLY. 
• The athlete expectation contract must be completed and turned into Coach Demere. 

PLEASE NOTE: DRAGONFLY is BEST navigated from a Desktop, Laptop or Chrome style computer.
