Physical & Parent Forms
Physical Information:
In order to participate in any sport, you MUST have a current physical on file. SEMS will be using the DRAGONFLY MAX app. Athletic forms must be completed electronically and uploaded on DRAGONFLY before a student can participate in any Athletics.
- The physical exam forms and medical eligibility, which must be completed by a physician, may be printed below. These forms must be uploaded directly to the DRAGONFLY MAX app.
- The travel/insurance form, cardiac awareness and concussion forms can be completed electronically on DRAGONFLY.
- The athlete expectation contract must be completed and turned into Coach Demere.
PLEASE NOTE: DRAGONFLY is BEST navigated from a Desktop, Laptop or Chrome style computer.
